- Risk: Discuss the probability and severity of the potential injury. List frequency, duration, # of employees exposed, stress factors, and most likely severity of potential injury.
- Recommendations: (Engineering controls, management controls, with options).
- Cost vs Benefits: (Use information provided in the course to estimate risk probability and severity, ROI, payback, etc.)
Assignment 2:
For this assignment, reference the accident scenario below and do the following:
1. Conduct surface cause analysis: Describe at least two hazardous conditions,
and/or unsafe behaviors/actions that directly caused or contributed to the accident.
2. Conduct root cause analysis: From the information gained in the surface cause
analysis, describe at least two possible system design weaknesses and/or system
performance failures that may have contributed to the surface causes.
Jack's hurt bad...
A call comes in from the worksite supervisor. Jack's been hurt. As an accident investigator, you immediately swing into action. You grab your investigator's kit and hurry to the worksite accident scene. By the time you get there, the EMTs are administering first aid. From initial statements, it appears that the scaffold was very unstable and caused Jack to lose his balance and fall. The scaffold was erected on muddy ground and one of the legs sunk into the ground. Jack was in a hurry while climbing the leg to get to the top of the scaffold. It's a serious accident so he will be transported to the hospital.

safety incentive/recognition program. Discuss why you believe the program is effective or ineffective. Is it proactive or reactive? What can be done to improve the program?
