Fatality Assessment and Control Evaluation (FACE) reports
This page includes links to Oregon workplace fatality reports in the National Institute for Occupational Safety and Health (NIOSH) and the Oregon Center for Occupational and Environmental Toxicology
(CROET) Fatality Assessment and Control Evaluation (FACE) systems. The reports include the date of the incident, a summary, incident details, description of the investigation, recommendations, and
other data.
Due to the time involved in reporting and investigating workplace fatalities and accidents, there can be substantial time lags between the incident and publication of the report on the FACE Web sites.
Not all workplace fatalities are investigated, and not all investigated fatalities appear in reports on FACE pages. If you have questions about specific workplace fatalities, please contact the OR-OSHA

FACE reports are for information, research, or occupational injury control only. Safety and health practices may have changed since the investigation was conducted and the report was completed. Persons
needing regulatory compliance information should consult the appropriate regulatory agency.
